Now enrolling your child(ren) in a CCLC program is even easier! Simply select your form, complete it, submit it and your Center Director will contact you to finalize your enrollment!

For families throughout the United States (except California) simply download and complete our editable enrollment form* and submit!

For families in California simply download and complete our editable enrollment form(CA)* and submit!

Before you begin be sure you have the following:

• Your child's doctor and dentist contact information
• Your child's immunization records
• The names and phone numbers for emergency contacts

If you have more than one child, after you click submit for the first child, you can edit the fields that need to be modified and leave the ones that are the same for all your children (such as address, etc).
