Enrollment

Enrolling your child in a CCLC program is easy! Simply choose your form, complete it, and submit it. Then, your Center Director will contact you to finalize your enrollment. Please note that enrollment is not guaranteed, so check with your Center Director before completing enrollment paperwork.

For families in the United States (except California and Texas), just download and complete our editable enrollment form* and click Submit.

For families in California, simply download and complete our editable enrollment form - CA* and click Submit.

For families in Texas, simply download and complete our editable enrollment form - TX* and click Submit.

Before you begin be sure you have the following:

  • Contact information for your child's doctor and dentist
  • Your child's immunization records
  • The names and phone numbers for emergency contacts

If you have more than one child: After you click Submit for the first child's enrollment, you can edit the fields that need to be modified and leave the ones that are the same for all your children (such as address, phone number, and so on).
