Ready to enroll?
If you’ve already spoken with a Director and he or she has provided you with a start date, you can download and complete our enrollment agreement by clicking on the appropriate link below.
Before you begin be sure you have the following:
- The name of the center you are enrolling in
- Contact information for your child's doctor and dentist
- Your child's immunization records
- The names and phone numbers for emergency contacts
